ΠΕΡΙΛΗΨΗ
Η γωνία της κάτω γνάθου είναι μία περιοχή ήσσονος αντίστασης σε κατάγματα και με ιδιαίτερη αισθητική
και λειτουργική σημασία.
Η θεραπευτική αντιμετώπιση των καταγμάτων της κάτω γνάθου και της γωνίας με τις μεθόδους ανοικτής
ανάταξης και οστεοσυνθεσης με πλάκες, έχει καθιερωθεί σχεδόν τα τελευταία τριάντα χρόνια, με
πολλούς όμως προβληματισμούς και αντιγνωμίες όσον αφορά τη μέθοδο της οστεοσύνθεσης και τον
αριθμό των πλακών.
Ο Champy είναι αυτός που τη δεκαετία του 70 θέλησε να καθιερώσει τη μέθοδο της μονοφλοιωτικής
οστεοσυνθεσης με μία «mini» πλάκα με πειραματικές και κλινικές μελέτες και με πολύ καλά αποτελέσματα.
Τη μέθοδο αυτή της μονοψλοιωτικής οστεοσύνθεσης υποστηρίζουν και άλλοι χειρουργοί πολλά χρόνια
αργότερα, τη δεκαετία του 90 και μέχρι τις μέρες μας, βασιζόμενοι στα χαμηλά ποσοστά μετεγχειρητικών
επιπλοκών των κλινικών τους μελετών.
Ο Luhr και οι άλλοι οπαδοί της συμπιεστικής οστεοσύνθεσης πιστεύουν ότι η μέθοδος αυτή εξασφαλίζει
την απόλυτη σταθερότητα της ακινητοποίησης των κατεαγότων οστικών τμημάτων και ικανοποιητική
επούλωση με το σχηματισμό του πρωτογενούς πώρου. Είναι γενικά παραδεκτό όμως ότι η μέθοδος της
συμπιεστικής οστεοσύνθεσης πρέπει να εφαρμόζεται με πολύ περίσκεψη στην περιοχή της γωνίας, όπου
οι δυνάμεις συμπίεσης σε συνάρτηση με τις τεχνικές δυσκολίες, μπορεί να προκαλέσουν εφίππευση ή
παρεκτόπιση των κατεαγότων οστικών τμημάτων.
Σε συντριπτικά κατάγματα της γωνίας ή σε κατάγματα με οστικά ελλείμματα, η εφαρμογή ισχυρών πλακών
αποκατάστασης οστικών ελλειμμάτων εξασφαλίζει ικανοποιητική σταθερότητα της ακινητοποίησης, με
χαμηλά σχετικά ποσοστά επιπλοκών. Έχει όμως το μειονέκτημα της εξωστοματικής προσπέλασης.
