Ψευδαγκύλωση της Κροταφογναθικής Διάρθρωσης. Εμπειρίες από Κλινικό Υλικό 7 Περιπτώσεων.

ΠΕΡΙΛΗΨΗ
Η εξωκαψική αγκύλωση της κάτω γνάθου προς το ζυγωματικό σύμπλεγμα συνήθως συμβαίνει μετά από
παραμελημένα, ατελώς αναταγμένα και σπανίως από ορθώς αναταγμένα κατάγματα του ζυγωματικού οστού
και τόξου. Επίσης μετά από φλεγμονές στην κροταφική και υποκροτάφια περιοχή, καθώς και μετά από εκτο-
μές όγκων στην περιοχή του πτερυγοειδούς βόθρου. Πρέπει ωστόσο να τονιστεί ότι πρόκειται για πολύ σπάνια
επιπλοκή. 'Οταν όμως συμβεί, συνήθως πρόκειται για ινώδη σύνδεση της κορωνοειδούς απόφυσης με
το ζυγωματικό τόξο. Ωστόσο, μερικές περιπτώσεις έχουν δείξει και οστική αγκύλωση ή ινοχόνδρινη σύντηξη
μεταξύ κορωνοειδούς απόφυσης και ζυγωματικού τόξου. Το δικό μας κλινικό υλικό αφορά 7 περιπτώσεις
ψευδαγκύλωσης, στη μία από τις οποίες έγκαιρα εφαρμόσθηκε έντονη κινησιοθεραπεία και δε χρειάστηκε
χειρουργική επέμβαση. Στους υπόλοιπους 6 πραγματοποιήσαμε κορωνοειδεκτομή με ενδοστοματική επέμβαση.
Τα αποτελέσματα υπήρξαν από ικανοποιητικά εώς πολύ ικανοποιητικά. Παρ' όλο που ορισμένοι αναφέρουν
και εξωστοματική προσπέλαση πιστεύουμε ότι η ενδοστοματική προτιμάται, διότι ακολουθείται από
ελάχιστη νοσηρότητα σε σύγκριση με μία εξωστοματική προσπέλαση για την τέλεση κορωνοειδεκτομής.
