Ανοσοϊστοχημική μελέτη των μεταστάσεων και μικρομεταστάσεων στους προσωπικούς λεμφαδένες σε ασθενείς με ακανθοκυτταρικό καρκίνωμα της στοματικής κοιλότητας

ΠΕΡΙΛΗΨΗ: Μία από τις ομάδες των λεμφαδένων κεφαλής και τραχήλου είναι και οι προσωπικοί
λεμφαδένες (ΠΛ). Αποτελούν μία ακόμη λεμφική ομάδα-στόχο για λεμφαδενικές μεταστάσεις σε
ασθενείς με ακανθοκυτταρικό καρκίνωμα της στοματικής κοιλότητας (ΑΚΚ). Σκοπός: Η παρούσα
ερευνητική μελέτη σκοπό είχε να μελετηθεί η συχνότητα των μεταστάσεων και μικρομεταστάσεων
στους ΠΛ σε ασθενείς με ΑΚΚ. Υλικό-Μέθοδος: Μελετήθηκαν ιστοπαθολογικά και ανοσοϊστοχημι-
κά 53 ΠΛ που ελήφθησαν από 43 ασθενείς που έπασχαν από ΑΚΚ για την ανεύρεση πιθανών μετα-
στάσεων και μικρομεταστάσεων, καθώς και η ιστιοκυτταρική λεμφαδενίτιδα που παρατηρήθηκε
στους αρνητικούς για μετάσταση ΠΛ. Αποτελέσματα: Μεταστάσεις παρατηρήθηκαν σε 6 ασθενείς
(13,95%). Βρέθηκε ότι η πιθανότητα μεταστατικής προσβολής των ΠΛ συνδέεται σε επίπεδο στατι-
στικά σημαντικό με το μέγεθος της πρωτοπαθούς βλάβης, την ύπαρξη κλινικά ψηλαφητών τραχηλι-
κών λεμφαδένων, το στάδιο της νόσου κατά το σύστημα ΤΝΜ και την ηλικία των ασθενών.
Συμπέρασμα: Η αφαίρεση των ΠΛ πρέπει να γίνεται με μεγάλη προσοχή καθώς βρίσκονται σε στενή
σχέση με τον επιχείλιο κλάδο του προσωπικού νεύρου. Σε ασθενείς με ΑΚΚ θα πρέπει να εξετάζε-
ται η πιθανότητα αφαίρεσής τους όταν υπάρχουν ψηλαφητοί τραχηλικοί λεμφαδένες, στάδιο νόσου
IV κατά ΤΝΜ, μέγεθος πρωτοπαθούς εστίας T4 και όταν η πρωτοπαθής εστία εντοπίζεται στον
βλεννογόνο της φατνιακής ακρολοφίας της κάτω γνάθου.
