SUMMARY: The intraoral approach for the removal of impacted third molars of the mandible is a common procedure in the everyday clinical practice. However in a few cases like ectopic third molars an extraoral approach is inevitable. Our case concerns a 50 year old male who had an impacted mandibular third molar together with a cystic lesion surgically removed via an extraoral submandibular approach. The postoperative course of the patient was satisfactory and without signs of paresis of the facial nerve. Appropriate planning with an extensive clinical examination as well as radiographic imaging is required for the appropriate surgical treatment plan to avoid any complications.
KEY WORDS: Ectopic mandibular third molar, cyst, extraoral approach.
