Posterior occlusal support is an important functional factor for a successful implant supported prosthesis. While tooth loss is inevitably followed by relevant bone loss, careful volumetric evaluation of the residual bone is the initial step in treatment planning the reconstruction. Utilizing the existing limited volume of bone of the residual ridge for supporting the implant prosthesis posteriorly, may reduce the need for bone augmentation by additional surgical procedures. Rehabilitation of the atrophic posterior maxilla can be accomplished by implant insertion simultaneously with closed or opened sinus floor elevation. Moreover, tuberosity/pterygoid Implants have also been applied as a successful alternative approach. On the other hand, reconstruction of the posterior atrophic mandible can be achieved by avoiding the mandibular nerve by deviating the implant insertion lingually or buccally on the cortical bone of the internal or external oblique line.
