Αγαπητά μέλη της ΕΕΣΓΠΧ,. αγαπητοί αναγνώστες των «Αρχείων»,
Η αυγή του 2024, συμπίπτει με προκλήσεις για την ιατρική και οδοντιατρική ειδικότητας της Στοματικής και Γναθοπροσωπικής Χειρουργικής. Με την Υπουργική Απόφαση ΥΑ Γ5α/Γ.Π.οικ. 7201/2024, αποφασίστηκε η μετονομασία της οδοντιατρικής ειδικότητας της «Οδοντοφατνιακής Χειρουργικής» σε «Χειρουργική Στόματος». Η μετονομασία αυτή δε συνοδεύεται από καμία ουσιώδη αλλαγή στην μεταπτυχιακή εκπαίδευση των συναδέλφων οδοντιάτρων οδοντοφατνιακών χειρουργών, εξ’ όσων είμαι σε θέση να γνωρίζω καθώς συμμετέχω στην εκπαίδευσή τους στο Γ.Ν.Θ. «Γ. Παπανικολάου». Κατά την προσωπική μου άποψη, το πιθανότερο είναι να προκαλέσει σύγχυση μεταξύ των ληπτών υπηρεσιών υγείας παρά να βοηθήσει στον αντικειμενικότερο προσδιορισμό του αντικειμένου των δύο οδοντιατρικών ειδικοτήτων, της Στοματικής και Γναθοπροσωπικής Χειρουργικής και της «Χειρουργικής Στόματος». Η Στοματική Χειρουργική ασκείται για περισσότερο από ένα αιώνα από Στοματικούς και Γναθοπροσωπικούς Χειρουργούς. Μόνο την τελευταία διετία, η στήλη «Ματιές στην Ιστορία της ιατρικής» υπό την επιμέλεια του Δρος Δρος Αναστάσιου Μυλωνά, δημοσίευσε βιογραφίες και έργα καταξιωμένων ιατρών-οδοντιάτρων, Στοματικών και Γναθοπροσωπικών Χειρουργών, όπως ο Simon Hullihen που θεωρείται ο πρώτος Στοματικός Χειρουργός (Oral Surgeon) στις ΗΠΑ, ο James Edmund Garretson που είναι αυτός που πρώτος χρησιμοποίησε τον όρο “Oral Surgery”, ο Richard Trauner, ο Robert H. Ivy, ο Norman L. Rowe, ο Truman W. Brophy και άλλοι οι οποίοι έμειναν στην ιστορία για την συστηματική καταγραφή και κοινοποίηση τεχνικών που στην πλειονότητα των περιπτώσεων αφορούσαν στη χειρουργική του στόματος. Η θεσμοθέτηση επομένως μιας οδοντιατρικής ειδικότητας, της «Χειρουργικής Στόματος» ενώ υπάρχει ήδη από το 2003 η ιατρική και οδοντιατρική ειδικότητα της Στοματικής και Γναθοπροσωπικής Χειρουργικής (Νόμος 3209/2003, ΦΕΚ 304 Α’), που εξ’ ορισμού το πρώτο συνθετικό της αφορά στη χειρουργική του στόματος, είναι πιθανό να προκαλέσει σύγχυση στην κοινότητα, αναφορικά με την διαχρονική αρμοδιότητα και επάρκεια των δύο οδοντιατρικών ειδικοτήτων.
Καλωσορίζω τον κ. Δημήτρη Τάτση (Google Scholar: 48 εργασίες, 169 αναφορές, δείκτης Hirsch=7), Στοματικό Γναθοπροσωπικό Χειρουργό και ήδη από 20 2021 βοηθό σύνταξης των «Αρχείων», στη νέα του θέση του αναπληρωτή Διευθυντή Σύνταξης. Ο κ. Τάτσης εξελέγη ειδικός γραμματέας της ΕΕΣΓΠΧ και είναι εκ του καταστατικού επιφορτισμένος με το συντονισμό του εκπαιδευτικού έργου της ΕΕΣΓΠΧ, επομένως η αναβάθμιση του αυτή είναι συμβατή με το ρόλο του στην ΕΕΣΓΠΧ.
Στο τεύχος που έχετε στα χέρια περιλαμβάνει δύο άρθρα ανασκόπησης. Το πρώτο προέρχεται από την Κλινική ΣΓΠΧ του ΕΚΠΑ, στο Γ.Ν. «Ευαγγελισμός» και αφορά στην παρουσίαση νέων δεδομένων και σύγχρονων διαγνωστικών μεθόδων στον τομέα της ορθογναθικής χειρουργικής [1]. Η 2η ανασκόπηση που δημοσιεύεται προέρχεται από το ΠΜΣ της Νοσοκομειακής Οδοντιατρικής του ΑΠΘ και αφορά στην οδοντιατρική διαχείριση των ασθενών με νόσο Parkinson’s [2]. Το τρίτο κατά σειρά άρθρο αφορά σε σειρά περιστατικών (case series) ασθενών με αντρόλιθους του ιγμορείου και προέρχεται από την Κλινική Οδοντοφατνιακής Χειρουργικής του ΑΠΘ [3]. Το τέταρτο άρθρο αφορά σε παρουσίαση περιστατικού σε ένα χρόνο πλαστικής αποκατάστασης του μετεγχειρητικού ελλείμματος σε επιπολής παρωτιδεκτομή και προέρχεται από το Τμήμα Στοματικής & Γναθοπροσωπικής Χειρουργικής της Κλινικής «Άγιος Λουκάς» της Θεσσαλονίκης [4]. Τέλος, στο άρθρο «Ματιές στην Ιστορία της Ιατρικής» ο Δρ.Δρ. Α. Μυλωνάς, αναφέρεται στο βίο και το έργο του Hayes Martin, ηγετική φυσιογνωμία της Χειρουργικής Κεφαλής και Τραχήλου που καθιέρωσε το λεμφαδενικό τραχηλικό καθαρισμό για όγκους της στοματικής κοιλότητας. The dawn of the year 2024 coincides with challenges for the medical and dental specialty of Oral and Maxillofacial Surgery in Greece. With the recent Ministerial Decision HY C5a/G.P.ok. 7201/2024, it was decided to rename the dental specialty of "Dentoalveolar Surgery" to "Oral Surgery". This renaming is not accompanied by any significant change in the post-graduate education of fellow dentists and dentoalveolar surgeons, as far as I am able to know as I participate in their training at the General Hospital "G. Papanikolaou". In my personal opinion, this decision is more likely to cause confusion among health care recipients than to help more objectively define the scope of the two dental specialties, Oral and Maxillofacial Surgery and "Oral Surgery". Oral Surgery has been practiced for more than a century by Oral and Maxillofacial Surgeons. In the last two years alone, the column "Glances in the History of Medicine" edited by Dr. Dr. Anastasios Mylonas, has published biographies and works of renowned dentists, Oral and Maxillofacial Surgeons, such as Simon Hullihen, who is considered the first Oral Surgeon in the USA, James Edmund Garretson who is the one who first used the term “Oral Surgery”, Richard Trauner, Robert H. Ivy, Norman L. Rowe, Truman W. Brophy and others who went down in history for the systematic recording and communication of techniques that in the majority of cases related to the practice of oral surgery. Therefore, the institution of a dental specialty, "Oral Surgery", while the medical and dental specialty of Oral and Maxillofacial Surgery has existed since 2003 (Law 3209/2003, Government Gazette 304 A'), which by definition, its first component concerns oral surgery, is likely to cause confusion in the community regarding the competence among the two dental specialties.
I welcome Dr. Dimitris Tatsis (Google Scholar: 48 papers, 169 references, Hirsch index=7), Oral and Maxillofacial Surgeon and as of 20 2021 editorial assistant of the "Archives", in his new position of deputy Editor in Chief. Dr. Tatsis was elected special secretary of HAOMS and is by statute charged with the coordination of the educational work of HAOMS, so his upgrade is very much compatible with his role in HAOMS.
The issue in your hands includes two review articles. The first one comes from the EKPA's OMFS Clinic, in the General Hospital "Evangelismos" and concerns the presentation of new data and modern diagnostic methods in the field of orthognathic surgery [1]. The 2nd review that is published comes from the Postgraduate Training Program of Hospital Dentistry of AUTH and concerns the dental management of patients with Parkinson's disease [2]. The third article is a case series of patients with androliths of the sinus and comes from the Dentoalveolar Surgery Laboratory of the Dental School of AUTH [3]. The fourth article concerns a case presentation of plastic restoration of the postoperative deficit in a superficial parotidectomy and comes from the Department of Oral & Maxillofacial Surgery of the "Agios Loukas" Private Hospital of Thessaloniki [4]. In the article "Glances in the History of Medicine" Dr. Dr. A. Mylonas, refers to the life of Hayes Martin, the leading figure of Head and Neck Surgery who first proposed and established lymphatic basin neck dissection for the treatment of oral tumors [5].
